| Description: | This position is a 1.0 FTE with daytime hours Monday - Friday.
This position is responsible for the care, treatment and patient education for elderly patients and their families. |
| Duties: | 1) Provide his/her patients with continuity of care at a professionally recognized level of quality and efficiency
2) Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene.
3) Requests necessary tests and follow-up visits and refers patients to specialists, as necessary.
4) Prepare and complete in a timely manner the medical records for all patients that he/she in any way provides care to in the Medical Center
5) Promotes an environment of teamwork, customer service and positive communications

| Qualifications: | EDUCATION
� Graduated from an accredited Adult/Gerontological Primary Care Nurse Practitioner program
� Valid American Academy of Nurse Practitioners (APNP) license
� Valid and current Wisconsin RN license
� Valid DEA certification.
EXPERIENCE
� 1 year experience as CNP preferred, inpatient or in a nursing home environment is ideal.
SKILLS
� Excellent communication skills with nursing home staff, patients, and other providers.
� Ability to educate elderly patients and their families regarding their medical conditions and prescribed treatments.
� Able to perform history and appropriate physical exam and order appropriate labs
� Able to formulate a diagnosis, and treat appropriately
