| Qualifications: | Nurse Practitioner Qualifications
� Graduated from an accredited Nurse Practitioner graduate education program
� Current certification with AANP or ANCC
� Valid and current Wisconsin RN license
� Valid and current APNP license
� Current or eligible for Wisconsin DEA certification.
Physician Assistant Qualifications
� Graduated from an accredited Physician Assistant graduate education program
� Licensed by the State of Wisconsin as Physician Assistant
� Certified by the NCCPA
� Current or eligible for Wisconsin DEA certification
